WELLNESS CONSULTATION CONSENT & DISCLAIMER AGREEMENT

  1. Purpose of Service
  • The purpose of this consultation is to provide general wellness education, lifestyle guidance, and health optimization coaching.
  • Topics may include nutrition, exercise, stress management, sleep optimization, lab interpretation education, and supplement or peptide information.
  • These services are not medical treatment and do not substitute for professional medical care provided by a licensed physician or healthcare provider in the Philippines.

 

  1. No Medical Diagnosis or Prescribing
  • The consultant is a U.S.-licensed Nurse Practitioner, not licensed to practice nursing or medicine in the Philippines.
  • All recommendations provided are for educational purposes only and are not intended to diagnose, treat, cure, or prevent any disease.
  • Clients are encouraged to consult with their local licensed physician or healthcare provider before implementing any recommendations.

 

  1. No Patient–Provider Relationship
  • Participation in this session does not establish a patient–provider relationship under Philippine or U.S. law.
  • The consultant acts as a wellness educator and coach, not as a healthcare provider in the client’s jurisdiction.

 

  1. Confidentiality and Data Privacy
  • All information shared is confidential and protected under the U.S. HIPAA Privacy Rule and the Philippine Data Privacy Act of 2012 (RA 10173).
  • Client data will be securely stored and never shared without written consent, except as required by law.

 

  • Client Acknowledgment and Responsibility:
  1. By signing or electronically accepting this agreement, the client acknowledges that:
  2. They understand this service is educational in nature.
  3. They will not interpret any recommendation as a prescription or medical directive.
  4. They accept responsibility for their own health decisions and outcomes.
  5. They have the right to seek independent medical advice at any time.

 

  1. Indemnification
  • The client agrees to hold the consultant harmless from any claims or damages arising from actions taken based on this consultation, to the fullest extent permitted by law.

 

  1. Consent to Proceed
  • By signing below (or clicking “I Agree” on an online form), I confirm that I have read and understood the information above, and I voluntarily consent to participate in this wellness consultation.
